Patient Card


Your patient card is an important part of your health care at NLC.
Please read the following points carefully
and bring your card with you to your next appointment.

Whenever you come to the clinic, for exams, tests or delivery, bring this card
with you and give it to the receptionist on arrival. Forgetting your card may
delay your appointment. Remember, you need to bring the card even if you
made your appointment by phone.
This card is for long-term use. Keep in a safe place so that it isn't lost.
There is a charge to reissue the card.
Please keep your card up-to-date. If there has been a change in your address
or place of work, please inform the receptionist when you arrive.
Remember to pick up your card when you leave the clinic.
Please bring your Japanese health insurance card with you once a month.
The number on the back of your card is your patient number. It will not
change as long as you remain a patient, NLC.
Have the card handy whenever you call the clinic.

The reception for general exams opens one hour before examinations begin.
Patients whose appointments can be made through the appointment-by-
phone system are encouraged to use it.
